Section 2799B-6 of the Public Health Service Act - you have the right to receive a GOOD FAITH ESTIMATE for health care items under the NO SURPRISES ACT LAW.
Patients are to receive an estimate of the bill for their medical services. You will receive this estimate before your scheduled service. If you at any point you receive a bill that is at least $400 above the good faith estimate, you can dispute your bill. For any questions, please visit- www.cms.gov/nosurprises..
In good faith, please note, that I am unable to provide an exact and definitive time frame of how long treatment will be. Treatment is unique to every individual, group, or family and may change over time per patients needs and as things come up. As my patient, you have the right to decide when treatment will be completed or finished.
