RATES

$215 per 50-min session
Sliding scale fee - limited number of space available for sliding scale.

INSURANCE

I am an ‘out of network’ provider (OON) and am not contracted with any insurance companies. If you have PPO insurance, I would be happy to provide you with a monthly super-bill that you can submit to your insurance for reimbursement. Some insurance companies reimburse, call your insurance provider to find out more info.

PAYMENT

I accept all major credit cards, cash,  and check as a form of payment.

CANCELLATION POLICY

If you are unable to attend or keep your appointment, please make sure you cancel 24-hours in advance, at least. If you cancel past the 24-hour mark, you may be charged the full rate of the session.

If you do not show up for your scheduled therapy session or attempt to reschedule your therapy appointment within the same day, and you have not notified me at least 24-hours in advance, you will be required to pay the full cost of the session.

If at any point you decided to take a break from therapy, or that we are no longer a fit, or you require a different type of therapy I ask that you kindly communicate with me so that I can provide you with the appropriate referrlas.

I do not provide refunds for therapeutic services.

GOOD FAITH ESTIMATE

As of January 1, 2022 clinicians are required to give a “good faith estimate” (GFE) to a patient of what services will cost and how long they may last. Section 2799B-6 of the Public Health Service Act - you have the right to receive a GOOD FAITH ESTIMATE for health care items under the NO SURPRISES ACT LAW.

Patients are to receive an estimate of the bill for their medical services. You will receive this estimate before your scheduled service.If you at any point you receive a bill that is at least $400 above the good faith estimate, you can dispute your bill. In good faith, please note, that I am unable to provide an exact and definitive time frame of how long treatment will be. Treatment is unique to every individual, group, or family and may change over time per patients needs and as things come up. As my patient, you have the right to decide when treatment will be completed or finished.
